Sculptra
Sculptra is a facial injectable that works gradually in a series of treatments for a subtle, more youthful looking appearance. Dr. Berkowitz uses Sculptra to target the underlying causes of the signs of facial aging. The Sculptra begins to work within the deep dermis, where your skin’s structure is reinforced and helps to replace lost collagen. This reinforced collagen structure provides a foundation that gradually restores the look of fullness of your shallow to deep facial wrinkles and folds that have depleted over time. Sculptra is different because it gradually and subtly corrects these facial wrinkles and folds. This can provide a refined, more youthful looking appearance that can last up to two years.

FOR CLOSE TO 10 years as a nurse injector it has been a privilege to work closely with Dr. Berkowitz and recently Dr. Zeidler. In the hands of a skilled clinician, the results can be beautiful and natural. Unfortunately, if overdone, lips may look too large for the face and detract from, rather than enhance a woman’s beauty. I enjoy discussing with our patients their ultimate goal for their dermal filler procedures, examining their unique anatomy of their face, and lips and choosing the appropriate dermal filler to achieve those desired results.

It is still exciting to me to transform a face to a more youthful appearance with judicious amounts of dermal filler carefully placed in the face and lips. Small amounts of dermal filler in the vermillion border, where the pink of the lips meets the skin around the lips, can effectively reduce the appearance of the fine lines above the lip without adding volume to the lips. Putting volume in the “pillows” or body of the lip can enhance the natural clefts that some lips have and create a more “pouty” look. Adding even amounts of dermal filler to the upper and lower lip can smooth out the texture and replace lost volume. There are fun, little techniques that can turn up the corners of the mouth or enhance the natural peaks or bows of the upper lip.

New advances make the field of injectable dermal fillers exciting, with more improved and predictable outcomes. Many of our dermal fillers now have longevity approaching or exceeding 1 year as compared with the injectable collagen of just a few years ago. The hyaluronic acid (HA) fillers, RestylaneTM and JuvedermTM, now have lidocaine in the product to reduce the discomfort of the procedure. And I am excited to be introducing the use of the Artiste® autoinjecting system, which ensures smooth and even injections of the dermal filler. This controlled injection system provides a carefully placed, exact amount of filler where you want it, eliminating the inaccuracies of a hand-held syringe. Also, microcannullas, used in place of needles may reduce trauma and the risk of intravascular injection during the procedures.

As we have previously written on the pages of this magazine, nothing can replace your own body fat as the ideal filler. However, the surgery required and attendant down-time make the hyaluronic fillers such as Restylane and Juvederm and the biostimulatory filler, SculptraTM a safe alternative for the patient who requires a rapid recovery or who is otherwise not a candidate for surgery.

As always, in our office we take extra care to reduce any untoward side effects of dermal filler procedures such as bruising, lumpiness and infection. Taking the time to properly anesthetize the treated area, prep the skin with a special antiseptic, and always using sterile latexfree gloves are some of the extra consideration I give our patients. No matter how simple these procedures are made to seem I never underestimate the seriousness of placing a substance in your face.

As with the change in fashion and hairstyles, I have seen a shift in the trends of cosmetic enhancements. Today’s patients are looking for refinement and correction of lost volume, with subtle but noticeable improvement in their looks.
