Why Lower Facelift Becomes Necessary

What to Expect During the Lower Facelift Procedure
Lower facelift surgery is performed with the use of either general anesthesia or local anesthesia with intravenous (IV) sedation. Before starting the procedure, Dr. Berkowitz might part the hair along the area where he will make the incision and draw the incision line with a marker, so he can properly position the incision in the way that will make the resulting scars inconspicuous.
To start the procedure, Dr. Berkowitz makes a single incision that starts at the temple and continues within the hairline down around the ear, terminating in the lower scalp region.
He uses surgical instruments to lift the skin away from the supporting tissues of the lower face, jaw line and neck, leaving the SMAS, or superficial muscular aponeurotic system, exposed. The SMAS is a layer of muscle and fibrous connective tissue, which Dr. Berkowitz may reposition to elevate and tighten the underlying facial structures.
Dr. Berkowitz then makes another incision on the underside of the chin, allowing him to reposition or remove neck musculature. This improves skin laxity and the appearance of horizontal neck bands that can make the neck look segmented.
Once he is satisfied that the underlying structures of the lower face and neck have been sufficiently tightened and repositioned, he pulls the residual skin taut and removes any loose excess skin. The surgeon may place surgical drains in the treatment area to help prevent the accumulation of fluid under the skin, after the procedure. Dr. Berkowitz closes the incisions with sutures and dresses the treatment area with soft bandages.
What to Expect During the Lower Facelift Recovery Period
Bandages and drainage tubes are removed within a few days following the procedure. Any non-dissolvable sutures are removed within seven to 10 days after surgery.
The treatment area might be bruised, swollen or sore, but these are normal symptoms that should gradually improve. Patients should be able to return to work or their normal schedule within one to two weeks. Often, immediate postoperative symptoms resolve in about one week, but it takes about two weeks for patients to be ready to venture out of the house without having any noticeable signs of having “work done.” During the initial recovery period, individuals should rest as much as possible and avoid strenuous activity, including heavy lifting and exercising.
Some of the aesthetic benefits of lower facelift surgery are visible as the immediate postoperative symptoms improve, but the rejuvenating effects of surgery will continue to improve over several weeks.
