What Is Fat Grafting?
Fat grafting is an alternative breast augmentation procedure that does not require placement of implants. NO CUTS, NO INCISIONS, NO IMPLANTS. Instead, fat is harvested from the patient’s own body and injected into the breast. Fat grafting is successfully used for breast augmentation, breast defect correction and breast reconstruction.
BRAVA/Autologous Fat Transfer (AFT) Method
“We do the fat grafting using the BRAVA external vacuum device to induce swelling in the breast for three weeks, followed by a round of fat grafting. We generally can get 50-70 percent of the fat to take. Often the patient will elect a second round.”
In Brava/AFT, the patient’s fat is harvested with liposuction as opposed to a scalpel; then it is injected into the breast area. Prior to this, the area has been prepped with an external tissue expander known as the Brava (a suction-pump bra).
"What I learned from my research was that all implants require changing — like tires — and general anesthesia and probably three surgeries would be required. I was looking for something more natural and less traumatic."
BRAVA was created by Dr. Roger Khouri in Miami for “breast regeneration.” BRAVA works by placing a gentle amount of tension (three-dimensional pull) on the breasts, and when sustained, this produces new breast tissue. This technique, known as tissue expansion, is not new — just the application is. For the last 30 years, the medical profession has used this same scientific principle to grow tissue for several medical procedures, including breast reconstruction and the lengthening of limbs. BRAVA, which holds numerous patents, is new because it uses this principle to non-surgically grow the soft tissue of the breast.
Dr. Berkowitz uses BRAVA to prep breast tissue for expansion, and then fills it with the patient’s own fat to sustain the new size and fullness.
“We have about five years of experience with fat grafting. I usually achieve at least a half and sometimes a full cup size enlargement. And, with a second round, we can double that, depending on how much fat a patient has to spare. That’s with NO incisions and NO implants. The fat is placed into the muscle and under the skin and it’s not put into the breast tissue itself.”

Should I Consider BRAVA/Fat Transfer?
Many women don’t want implants or a major surgery. Some patients don’t have the fat in the right spot. The beauty of this method is that it allows natural body sculpting. It doesn’t matter where the patient has excess fat — the belly, the back or the thighs.
Consider fat transfer if you seek:
- To achieve fuller and/or bigger breasts
- To preserve your own breast tissue
- To avoid the risks and maintenance of breast implants (capsular contracture, shifting, implant monitoring and replacement)
Also, you must be willing to use the BRAVA device up to 10 hours a day (to learn more about BRAVA visit www.mybrava.com).
“Dr. B and his staff were flawless throughout the entire process. From my first consultation; to pre-op, operation, and post-op. Dr. B’s bedside manner is unparalleled to any other experience I have ever had. He called me from home every night for the first week to make sure I was ok, he genuinely is a sweet man and wants nothing but perfect results for his patients. He had a “cooling system” device delivered to my home, so I could ice my chest throughout my recovery. He also prescribes a medication that you take before surgery, so you don’t come out of anesthesia feeling like you got hit by a bus. I felt so safe and comfortable with his staff, everyone from front office staff to his nurses; Kathy, Sabrina, Cecilia, and Virginia are AMAZING & delightful!!! I feel like I am a part of their family and will never forget this experience as long as I live. It was very refreshing and humbling that a plastic surgeon encouraged me to go the NATURAL route. I finally feel like I wasn’t just another dollar sign to a doctor. Less is more ladies… Au Naturel. : ) ”
